The Transitional Care Unit (TCU) is a 43-bed unit for Alternate Level of Care (ALC) patients. There are ten semi-private rooms and six rooms with four beds. ALC patients are placed in this unit because they have improved and no longer need acute care treatment.
Therapies offered on the TCU are physiotherapy, occupational and speech language therapy. The goal of therapy is to maintain/optimize your current level of functioning. Exercise and dining programs are held during the week.
Visiting hours are from 11:30 a.m. to 9:00 p.m. Patients may have two visitors at any one time. Click here for more information about visiting the Health Centre.

Discharge Options
Long Term Care (LTC)
LTC is a permanent residence for a patient who is unable to return home. Nursing care is provided. Previously referred to as Nursing Home.
Complex Continuing Care
A residence for patients who are no longer acutely ill but need more specialized medical/nursing care than is offered in LTC. Patients may return home.
Convalescent Care
A facility that offers a short stay for patients who are discharged from hospital but require additional support for medical problems prior to returning home.
Home
Supports from community agencies may help patients to return home to live independently.
Palliative Care
A facility that provides end-of-life care.
Rehabilitation Care
A program that offers physio and occupational therapy for patients who demonstrate the potential for recovery. Qualifying patients must be able to participate in at least 30-45 minutes of therapy a day.

What is Co-Payment?
The Ministry of Health mandates the Co-Payment for ALC patients in the hospital. The maximum amount you will pay is currently $48.69/day. This is the same fee you will be paying in Long Term /Complex Care. For more information contact Patient Accounts at ext: 3096.
What is a Power of Attorney (POA)?
The POA is the person legally designated by a patient to handle that patient's financial and/or personal care decisions. The POA assumes this responsibility when the patient is no longer capable.
What is an Advance Directive?
Advance Directives allows you and your family to set limits and have greater control over health care decisions. If you have prepared a document naming someone to speak on your behalf when you are unable to do so, have an advanced directive, or have a living will, please inform a member of your health care team on admission to the TCU.
What does ALC mean?
An Alternate Level of Care (ALC) patient has finished the acute care phase of treatment. You will stay at St. Josephs Health Centre until the discharge plan is finalized with the health care team.
What do volunteers do on the Transitional Care Unit?
You may see volunteers at different times during visiting hours. Out volunteers brighten your stay by providing friendly visits.

How can families get involved on the Transitional Care Unit?
St. Joseph's Health Centre believes in patient centered care, which includes family involvement. You can help by being present at meal times, assisting with basic care, bringing in reading material, music etc. Please ask any member of the team how you can participate in care.
What personal belongings do you need?
Please bring comfortable clothes and non-slip footwear to participate in activities. Feel free to bring personal care items such as, toothbrush, Kleenex etc.

The public washroom is located across from the Nursing Station and in the patient lounge.
