Heart Failure Clinic

Heart Failure is a common medical condition affecting more than 400,000 Canadians. Today, heart failure can be well managed, especially if you take an active part in your care.

The Heart Failure team plans to provide a series of educational classes for patients and their families to better understand the condition and decrease the need for hospitalization. The interprofessional team assesses and recommends treatment for patients diagnosed with Heart Failure.

The clinic's team consists of a Cardiologist, Registered Nurse, Registered Dietician, Registered Pharmacist, Physiotherapist and a Social Worker.

Patients should bring their hospital blue card, health card and ALL medications, vitamins and supplements in their original bottles, not just a list of medications. All applicable diagnostic/lab reports should be faxed with your referral prior to your booked appointment.

The clinic booking office is open from Monday to Friday between 7:30 a.m. to 4:30 p.m. Please call 416-530-6043 to make an appointment or for more information.

The Heart Failure Clinic is located at St. Joseph's Health Centre, in the Ambulatory Care Centre, Ground Floor, Glendale Wing. Please register for each visit in the Registration office, located on the ground floor of the hospital, by the Tranquility Entrance.

What number do I call if I need to change or cancel my appointment?
You may call the clinic booking office at 416-530-6043, from Monday to Friday between 7:30 a.m. - 4:30 p.m. Please give us a minimum of 48 hours notice for cancellations, so we can offer another patient the appointment time.

What is Heart Failure?
Heart Failure (HF) usually develops gradually. It is a condition in which the heart does not pump as strongly as it should. The body does not get the right amount of blood and oxygen it needs to work properly. The weakened pumping action can cause a back-up of fluid (congestion) in the lungs and other parts of the body. Without a proper oxygen supply and with congestion, you may feel tired and short of breath at times. There are other symptoms of Heart Failure that you may have experienced as well.

Why do I need to be referred to the Heart Failure Clinic?
By meeting with our Heart Failure Clinic team, you will become more informed about your condition. It is our hope that with this information, you will be able to manage your care and thus, you will feel better. In assisting you to manage your care more closely, we will strive to decrease the need for hospital admission.

Who are the people on the Heart Failure Clinic team?
The team is composed of the following health professionals:

What should I expect at my first appointment?
You will need to register before your appointment, so please arrive at least 20 minutes early. During your inital visit you will meet with the members of our Heart Failure Clinic team. The appointment may last up to 3 hours during which time several diagnostic tests that may be performed, such as blood tests, an electrocardiogram, an echocardiogram and chest X-ray. A series of education classes will be provided by the members of our Heart Failure Clinic team to help you better understand your situation. Your family are also welcome to attend these classes.

What do I need to bring with me to my appointment?

Should I bring someone with me to my clinic appointment?
We strongly encourage a family member to accompany you to your appointment. We are also able to arrange for an interpreter to meet with you and the Clinic team, if you wish. Please advise our booking office at least 72 hours prior to your scheduled appointment if you require an interpreter.

Where is the clinic located?
The clinic is in the Ambulatory Care Centre. Please register for each visit in the Registration office, located on the ground floor of the hospital, by the Tranquility Entrance.
