Anticoagulation Therapy Clinic
The Anticoagulation Therapy Clinic is located at St. Joseph's Health Centre on the ground floor in the Glendale Wing.
Our services include
- Patient assessments
- Review of current medications, over-the-counter medications, herbals and supplements to check for possible drug interactions
- Ongoing patient education
- Monitoring of lab results
- Dosage adjustments as necessary
- Communication with family physician and/or specialist
Patients should be referred to the Anticoagulation clinic if they have been prescribed warfarin (Coumadin®). All patients need to be referred by their family physician or specialist. Ask your physician to fill out a referral form and fax it to the Anticoagulation Clinic. You will then be contacted to book an appointment.
Patients need to visit the Registration Office before their first appointment with us. The registration office is located on the ground floor of the hospital, by the Tranquility Entrance.
When visiting the clinic patients should bring their health card, St. Joseph’s Health Centre blue card (if available) and all medications (not just a list), including over-the-counter medications, vitamins, herbals and supplements.
On every visit, the pharmacist will ask you for your health and medical history, including your eating and exercise habits. Your INR will be tested to determine how long it takes for your blood to clot. The pharmacist will then discuss the results and your new warfarin (Coumadin®) dosage if necessary. Since everyone responds differently to warfarin (Coumadin®), more frequent visits may be needed to determine the correct dosage of warfarin (Coumadin®) for you, initially. Once the dosage has been stabilized, visits will be every four to six weeks.
A Registered Pharmacist operates this outpatient clinic.
