This new site operates in coordination with St. Joseph's Health Centre's current Family Medicine Centre, providing comprehensive care for patients and their families. UFHT patients receive care from an interprofessional team made up of Family Physicians, Nurse Practitioners, Pharmacists, Dietitians and Social Workers.
This interprofessional team gives patients access to chronic disease management programs, health promotion and disease prevention programs.
It also connects people with education programs, enhanced home visits, services for frail elderly and palliative care patients, improved service coordination and referrals, case management and specialized access to pharmacist services.
Building on St. Joseph's Vision to be Canada's Best Community Teaching Hospital, the UFHT plays a key role in the training and development of the next generation of primary care professionals. This teaching and education component will help improve access to high quality services for members of our diverse communities. This site continues to provide learning opportunities for the Family Medicine residents from the University of Toronto and other students from different health professions.

The St. Joseph's Urban Family Health Team's hours of operation are 9:00 a.m. to 5:00 p.m. Monday to Friday, closed each day between 11:45 a.m. to 12:45 p.m. for lunch.

Are you a patient of Family Medicine/Urban Family Health Team ? Would you like to get your Seasonal Flu Shot this year ? Here's what you can do: #1 Simply call 416-530-6860 or 416-530-6947 to book you and your family members in any of our Seasonal Flu Shot Clinics.
These are scheduled as follows: #2 If you are one of our patients you can also come to our Flu Shot Drop-ins starting Friday, Nov 9 between 13:00-16:00hrs at our 30 Queensway site with our Nursing Team to get your seasonal flu shot. #3 Whenever you are coming for your follow-up appointment, ask our staff and one of our nursing team members will give you the seasonal flu shot as part of your visit.
