This new site operates in coordination with St. Joseph's Health Centre's current Family Medicine Centre, providing comprehensive care for patients and their families.
UFHT patients receive care from an interprofessional team made up of Family Physicians, Nurse Practitioners, Pharmacists, Dietitians and Social Workers.
This interprofessional team gives patients access to chronic disease management programs, health promotion and disease prevention programs.
It also connects people with education programs, enhanced home visits, services for frail elderly and palliative care patients, improved service coordination and referrals, case management and specialized access to pharmacist services.
Building on St. Joseph's Vision to be Canada's Best Community Teaching Hospital, the UFHT plays a key role in the training and development of the next generation of primary care professionals. This teaching and education component will help improve access to high quality services for members of our diverse communities. This site continues to provide learning opportunities for the Family Medicine residents from the University of Toronto and other students from different health professions.

The St. Joseph's Urban Family Health Team's hours of operation are 9:00 a.m. to 5:00 p.m. Monday to Friday, closed each day between 11:45 a.m. to 12:45 p.m. for lunch.
