Quality Improvement Plan 2012-2013

Quality improvement is an ongoing priority that helps us continually find new and better ways of doing things so that we can enhance care for our patients. It gives our organization the ability to very clearly see our targeted areas for improvement and chart our progress. It also provides us with a meaningful way for to clearly articulate our accountability to our community, patients and staff.

We’ve had some great success stories in quality and patient safety over the last two years. Thanks to the hard work of all of our staff and physicians we’ve:

  • Reduced C. Difficile over the past two years by 35%.
  • Reporting of patients experiencing a Pressure Ulcer while in our care has increased by 21% in two years, however there has been an estimated 11% reduction in the severity of the Pressure Ulcers reported.
  • Hand hygiene compliance has increased from 38% to 91% in the past year.
  • In the past two years, patient’s experiencing a Fall with Harm while in our care have been reduced by 29%.
  • Medication errors with high risk medications, has decreased by 26%.
  • Our total margin, which reflects the financial well-being of the Health Centre, is positive.
  • Patient satisfaction in the Emergency Department is over 83%.

We are in year two of our Quality Improvement Plan and St. Joe’s will continue to build on our previous work and successes while stretching the organization to be a top performer in the priority areas we’ve identified that will enhance patient safety.
