The following sets out admission criteria for physicians referring patients to St. Joseph's Health Centre.
Cystoscopy
All procedures in the Cystoscopy Unit are scheduled after a consultation appointment with the urologist. To refer a patient, please call the specialist's offices directly. Clinic hours are Monday to Friday, 7:30 a.m. to 3:30 p.m.
Elderly Community Health Services
Written referral is required from a physician, nurse, physiotherapist, occupational therapist or speech language pathologist. Patients must be over 65 years of age and have been assessed by a clinician as requiring the services of a geriatrician. Clinic hours are Monday to Friday, 8:00 a.m. to 4:00 p.m.
Fracture Clinic
Patients must be referred from the Emergency Department at St. Joseph's Health Centre, or by an inpatient department, an orthopaedic surgeon, a family physician in our catchment area, or other physician. Patients must meet one of the following criteria:
- recent soft tissue injuries or acute inflammatory joint problem
- any fracture requiring ongoing care regardless of timing of injury
- back injuries if diagnosed as a fracture or nerve injury
- patient less than three months of age with foot or hip deformity
- flare-up of previous problem within three months of discharge from clinic
- acute joint injuries, i.e. ankle and knee ligament injuries or hemarthrosis
Prior to referring a patient to the Fracture Clinic at St. Joseph's, please call 416-530-6145. Clinic hours are Monday to Friday, 7:30 a.m. to 5:00 p.m.; weekends 7:30 a.m. to 11:30 a.m.
GI Unit
All procedures in the G.I. Unit are scheduled after a consultation appointment with a specialist. To refer a patient, please call the specialist's office directly. Clinic hours are Monday to Friday, 7:00 a.m. to 4:00 p.m.
Haematology/Oncology Clinic
An interdisciplinary team provides care to patients over 16 years of age who require specialized diagnostic, palliative or therapeutic care. Consultation services are provided for all patients, however not all patients are treated at St. Joseph's. An oncologist will assess whether a patient should be seen at our Health Centre or referred to another facility. Oncologists work in collaboration with family physicians when treating patients. To make a referral, please contact the oncologist's office directly.
Rehabilitation
St. Joseph's offers cardiac rehabilitation, occupational therapy, physiotherapy and speech-language pathology. To refer a patient, a completed and signed rehabilitation referral form or a doctor's written order/prescription is required. To receive a referral form, please call 416-530-6058. Clinic hours are Monday to Friday, 7:30 a.m. to 5:00 p.m.
Renal Therapy
All referrals to the Renal Therapy Centre are made through the nephrologist's offices with patient information faxed to 416-530-6533. The Centre is open Monday to Saturday, 6:30 a.m. to 10:30 p.m.
Withdrawal Management Services
Patients, 16 years of age or older, is admitted to St. Joseph's Withdrawal Management Services if intoxicated, in chemical abuse withdrawal, or in crisis related to substance use, except under the following circumstances:
- patient is unconscious or non-ambulatory
- physical or mental health gives program staff reason for immediate concern
- person poses a threat to the safety of self, staff or other clients in the service
- there are no available beds
- patient is unwilling to be searched for alcohol, other drugs, or weapons
- patient is unable to self administer approved medications
