St. Joseph’s Health Centre (SJHC) is subject to the Freedom of Information and Protection of Privacy Act (FIPPA) as of January 1, 2012. This statute promotes the privacy of individuals and provides a right of access to records in our custody or under the control of the hospital on or after January 1, 2007, with some restrictions. We promote an environment of openness and transparency within the community.
Information you may have access to:
- Your own personal information
- General records, such as administrative, operational records, and expenses
Correction to your own personal information:
- You may request a correction to your own personal information
Types of hospital information or records excluded from FIPPA*:
- Quality of Care information (as defined in the Quality of Care Information Protection Act)
- Ecclesiastical records
- Operations of the hospital foundation
- Administrative records of a member of a health profession that relates to their personal practice
- Certain research records
- Charitable donations made to the hospital
- Records relating to appointments and privileges
- Certain teaching materials
- Certain labour relations or employment-related matters

Right of Appeal
If you are not satisfied with the decision of the hospital concerning your request, you may contact the Office of the Information and Privacy Commissioner of Ontario
