A Roadmap for Co-Creating Interprofessional Models of Care

Interprofessional practice occurs when different professions come together for the purpose of patient-centred healthcare delivery. The purpose of this Toolkit is to provide leaders and clinicians working in a variety of healthcare settings with a set of guidelines and tools that will enable the enhancement and/or development of interprofessional teams.

The Toolkit provides a framework which organizations and individuals can use to engage with one another in the co-creation of interprofessional models of care and can be used to help move interprofessional practice from conceptualization to actualization.
